I am a doctor by profession, but I am writing these words first and foremost as a grateful patient. I was diagnosed with complex metastatic melanoma in the head and neck area, with in-transit metastases. After starting the workup and treatment in the center of the country, I came, following a warm recommendation, to Dr. Meni Bracha at Hadassah. Today I can say that the decision to move to his care was one of the most significant and best decisions I made in dealing with the disease. There are many things for which I want to thank him, but I want to begin specifically with the thinking behind the treatment. In my case, his understanding of melanoma and the biology of the disease was expressed in practical and precise decisions, with planning several steps ahead. After a discussion by the broad team at Hadassah, a neoadjuvant approach was chosen for me, meaning treatment before surgery. Even before the treatment began, in a creative move by Dr. Bracha, special marking of the in-transit metastases was performed, as part of the planning for the surgery that would follow. After two rounds of treatment that went well, I was operated on by Dr. Bracha. The significance of the marking became clear later on. In some of the sites there was an excellent response to the treatment, to the point that it was very difficult to identify the location of the original lesions. Thanks to the prior marking, it was possible to precisely locate the appropriate areas and excise the tissues for pathological examination even in places where the response to treatment was very significant. This made it possible to assess the extent of the response and obtain information that influenced decisions regarding the continuation of my treatment. As a doctor, this is one of the things that impressed me especially. Dr. Bracha planned in advance not only how to perform the surgery, but also how it would be possible to find the right areas and assess the response if the treatment succeeded greatly and the lesions became difficult to locate. He took into account, already at the beginning of the process, the information that would be needed later on. In my eyes, this was brilliant clinical thinking. Not a minor technical detail, but a precise connection between the understanding of the disease, the treatment given before it, the surgery, and the decisions that would follow. My case demonstrated to me how significant it is to be treated by an oncological surgeon with a deep understanding of melanoma, who knows how to translate it into a practical plan for the specific patient in front of him. The surgery itself went excellently. Dr. Bracha personally performed the excision (which included removal of metastases as well as a lymph node in the neck) and also the reconstruction required at one of the surgical sites using a skin flap. For me, it was very significant that the same surgeon combined oncological thinking with the ability to also perform the reconstruction. As I understand it, this is a unique thing in our small country, in the field of melanoma surgery, where there is one specialist who addresses all the complex surgical aspects. But this is certainly accepted at leading cancer centers around the world, and not without reason. I was very impressed by his precision, skill, and the range of his surgical abilities. The cosmetic result was perfect as far as I am concerned. When it comes to surgery in the head and neck area, this is a matter of great personal significance, alongside the oncological goal for which I underwent the treatment. But this testimonial would not be complete without talking about his humanity. Throughout the entire process, Dr. Bracha was attentive, caring, and exceptionally accessible. He was available to me on his personal phone at all hours of the day and night, and responded to every question and every issue, even the smallest one. I felt that I could reach out to him and that I would receive a serious and human response. During a period of dealing with such a diagnosis and all the uncertainty surrounding it, knowing that there is a doctor who knows the case in depth and who can be reached is an enormous thing. For me, his availability and attitude were an integral part of the sense of security I felt throughout the treatment. I know the world of medicine from the inside, and in this case I had the privilege of experiencing it also from the patient's side. The combination I encountered in Dr. Bracha of judgment, oncological understanding, surgical talent, and humanity was exceptional in my eyes. I consider him a gifted surgeon and a doctor I would want more patients to know about.
